J. Stark Munro recounts the beginning of his medical career in a sequence of letters to his friend Herbert Swanborough. After qualifying, Munro joins the brilliant, domineering and unpredictable Dr Cullingworth, whose energy and confidence initially promise an exciting partnership but gradually reveal deep differences of temperament, ethics and judgement.
The letters follow Munro through professional disappointment, financial uncertainty, friendship and the effort to establish an independent practice. Drawing closely on Conan Doyle’s own early medical experiences, the novel combines autobiographical material with arguments about medicine, belief, ambition and character.
